4-STAR AND 5-STAR CUSTOMER SERVICE HOSPITAL AWARD

Community Hospital Honored as PRC Four- and Five-Star Customer Service Hospital Scottsdale,
AZ June 5th and 6th, 2005 — Community Hospital was named as a 2005 PRC 4-Star
and 5-Star Customer Service Hospital at an awards luncheon held at The Hyatt
Regency Scottsdale at Gainey Ranch. Hospital leaders from across the United
States gathered here to participate in the 2005 PRC Client Education Conference
hosted by Professional Research Consultants, Inc. Community Hospital was awarded for its high scores in its employee perception
survey and patient survey. Awards received were:

The 5-Star Award is the designation given annually to any healthcare facility
that scores in the top ten percent (at or above the 90th percentile) of PRC’s
national employee and/or patient perception database for the prior year. It is
based on the percentage of the facility’s employees or patients who rate their
facility “excellent” for any of the ten select standard PRC questions. The facilities receiving the 5-Star award not only receive year-long
recognition, but also serve as benchmarking resources for all PRC clients.
4-Star Awards are given to any healthcare facility that scores between the 75
and 89.9 percentile PRC’s national employee and/or patient perception database
for the prior year. It is also based on the percentage of employees or patients
who rate their facility as “excellent” for any of the ten select standard PRC
questions. Employee and patient research is conducted to obtain the opinions of the
employees and patients at Community Hospital via telephone for a confidential
interview to assess their perceptions of hospital operations and patient care.
The research findings are then presented to the hospital to use in continuous
improvement and enhancement of employee satisfaction and patient satisfaction.
