The qualified staff of Community HealthCare
and Hospice helps
people creatively face the challenge of chronic illness or
aging in their own home.
Through individualized services, we work closely with
the physician and family to provide total care. A nurse is
available by phone, to provide full 24-hour coverage and
address the needs of any client at any time.
A professional staff nurse has primary responsibility
for each client, ensuring that each individual's care is
personalized and beneficial regardless of the length of
care or changes which may occur in his or her prescribed
treatment.

Hospice Care Services is a
philosophy of care for individuals with terminal or
life-threatening illnesses, and is intended to provide
comfort and relief from pain - physically, emotionally,
and spiritually. It is based on the belief that these
special people should be able to live their lives to the
fullest extent possible. They, along with the members of
their families, receive care to preserve and enhance
their dignity and their sense of personal fulfillment.
Hospice Care is available in homes, nursing homes, and
hospitals - wherever it is needed.
In order to reach these goals, Hospice Care includes the
full range of Community HealthCare and Hospice services listed
above, along with a number of additional services;

Hospice Care Reimbursement
Hospice services may be covered by Medicare with
expanded benefits to
include medications, homemaker services, equipment,
oxygen, nursing and home
health aide services, physical therapy, occupational
therapy, speech
therapy, social work, respite, volunteers, chaplains,
counseling (such as
nutrition and pharmacy), bereavement and extended hours
of care during a
crisis period. Private payment (according to ability to
pay) and private
insurance are other ways that Hospice services may be
covered.

Lifeline Program
You want to live your life your way - this includes
living in the comfort of your own home surrounded by
memories of a lifetime. Lifeline allows you or your
loved one to live at home because help is as close as
pressing a button. Lifeline provides 24-hour, 7-day
communication that can be activated whenever you may

Home Health
and Hospice services are available in Red Willow,
Frontier, Hayes, Hitchcock, Furnas, Dundy and Chase
counties. Hospice services are also available in Kansas
